We connect you with suitable providers, assist with service agreements, set up service bookings, and coordinate your supports across all relevant areas of your plan.

Are You Eligible for NDIS Support Coordination?

To receive NDIS Support Coordination through Connect Support Group, participants must meet the following criteria. We support participants of all ages across Victoria and throughout Australia.

Eligibility Requirements

Active NDIS Plan: You must have a current NDIS plan that includes Support Coordination as a funded support under the Capacity Building — Support Coordination budget category.
NDIS Participant: You must have met the NDIS access requirements and be an active NDIS participant, aged under 65 at the time of your first NDIS plan.
Location: We provide in-person support across Victoria and coordination services to participants anywhere in Australia. Contact us to discuss your location and how we can best support you.

Support Coordination not in your plan? If you believe you need Support Coordination but it is not currently funded, you can request it at your next NDIS plan review. Contact us — we can help you prepare the evidence needed to make this request.

What does a support coordinator do?

A support coordinator helps NDIS participants implement their NDIS plan by connecting them with service providers, assisting with service agreements, managing budgets across funding categories, and ensuring supports are working toward their plan goals.

At Connect Support Group, our coordinators handle provider sourcing, scheduling, administrative tasks, plan review preparation, and ongoing monitoring — so participants can focus on their goals rather than the paperwork. We work across all supports in a participant’s plan, including allied health, community access, housing, and daily living.

How much does NDIS Support Coordination cost?

NDIS Support Coordination is funded by the NDIS and does not cost participants anything out of pocket, provided it is included in your NDIS plan. The NDIA sets maximum prices through the NDIS Price Guide each financial year.

As of the 2024–25 NDIS Price Guide:

Connect Support Group charges within NDIS Price Guide limits. There are no hidden fees. A written service agreement outlining all costs is provided before services commence.

What is the difference between Support Coordination and Specialist Support Coordination?

The NDIS funds two levels:

Support Coordination (07_002_0106_8) is available to most NDIS participants who need help implementing their plan and connecting with providers. It is suited to participants who need general navigation and service coordination support.

Specialist Support Coordination (07_004_0132_8) is a higher-intensity service for participants with complex, high-risk, or crisis-level needs. It requires practitioners with specialist qualifications (social work or occupational therapy) and is funded when a participant’s situation involves significant barriers, multiple service systems, or crisis risks.

Both are funded under the Capacity Building — Support Coordination budget. Neither is available unless it appears as a line item in your current NDIS plan. Connect Support Group offers both levels.

How do I know if I have Support Coordination in my NDIS plan?

Log in to the myplace participant portal at my.ndis.gov.au and check your Capacity Building budget. If Support Coordination appears as a funded line item (07_002_0106_8 or 07_004_0132_8), you are eligible for this service.

You can also contact the NDIS on 1800 800 110, speak with your Local Area Coordinator (LAC), or contact Connect Support Group directly and we will help you understand your plan.

Can I change my support coordinator?

Yes. NDIS participants have the right to choose and change their support coordinator at any time. To change providers, you (or your plan manager) sign a new service agreement with your chosen provider and end the existing service agreement with your current provider.

You do not need NDIS approval to change support coordinators. Contact Connect Support Group to discuss switching — we make the transition as smooth as possible.

Am I eligible for Support Coordination?

To be eligible for NDIS Support Coordination through Connect Support Group, you must:

Have an active NDIS plan that includes Support Coordination in your Capacity Building budget
Be an active NDIS participant (aged under 65 at time of first plan)
Be located anywhere in Victoria or Australia — we provide in-person support across Victoria and can arrange coordination services nationally

Not all participants have Support Coordination in their plan. If you need it but don’t currently have it, you can request it at your next plan review. We can help you prepare this evidence.

What is the NDIS line item for Support Coordination?

The NDIS line item for Support Coordination is 07_002_0106_8, funded under the Capacity Building — Support Coordination budget. Specialist Support Coordination is line item 07_004_0132_8, also under Capacity Building — Support Coordination.

Neither line item draws from your Core Supports budget. They are separate, dedicated funding lines.

Do you support people with autism (ASD)?

Yes. Connect Support Group has experience supporting NDIS participants with Autism Spectrum Disorder (ASD) across all ages — children, adolescents, and adults. We understand autism-specific support goals and can connect participants with ASD-specialist therapists, allied health providers, community participation programs, and employment supports across Victoria and nationally.

How long does Support Coordination last?

What areas does Connect Support Group service?

Support Coordination is funded within each NDIS plan period, which typically runs for 12 months. At each plan review, the NDIA decides whether to continue funding Support Coordination based on participant progress and ongoing needs.

Your Connect Support Group coordinator will assist you in preparing evidence and documentation for your plan review to support continued funding.
