Assistance with Daily Life
Short notice cover so a hospital discharge is not held up waiting on supports.

Short Notice Support

Hospital Discharge Support

Leaving hospital is often the point where daily living support is needed fastest and planned least. We can step in at short notice so a discharge is not delayed, and scale the hours back once you have settled.

1

Immediate support at short notice

Cover arranged quickly when a discharge date moves or arrives unexpectedly.

2

24 hour care through the adjustment period

Higher intensity cover for the first days or weeks at home, then stepped down.

3

Longer term high level support where needed

Ongoing arrangements built around individual needs rather than a fixed package.

4

Medication, mobility and household tasks

The practical assistance that makes staying home safe after a hospital stay.

5

Coordination with your health professionals

We work with treating clinicians so care continues rather than restarts.

Our Process

How Support Gets Started

Three steps from first phone call to a worker at your door, with your preferences recorded in writing before anything begins.

1

Free Consultation

We talk through your plan, the tasks you want help with, your routine, and any language or cultural preferences. There is no cost and no obligation.

2

Service Agreement and Matching

We set out the supports, hours and rates in a written service agreement, then match you with workers who suit your needs and preferred language.

3

Support and Review

Support starts on the agreed schedule. We check in regularly, adjust hours as your needs change, and keep records that help at your next plan review.

Assistance with Daily Life
Personalised care and support, agreed in writing before the first shift.

Flexible and goal oriented

We work flexibly with you to help you reach your objectives and make the most of your NDIS funding. That means shifting hours when your week changes, building your skills in tasks you want to take on yourself, and being upfront when something would be better funded a different way.

If you already have a provider and want to change, you can. Participants choose their own providers under the NDIS. You end your current agreement according to its notice period, sign a new one with us. No NDIA approval is required.

Questions Participants Ask

Frequently Asked Questions

Straight answers on what this support covers, what it does not, and what it costs. If your question is not here, call (03) 9874 8628.

Assistance with Daily Life is a support category inside the NDIS Core Supports budget. It funds a support worker to help with everyday tasks a participant cannot do on their own because of disability, including showering, dressing, grooming, eating, medication prompting, cleaning, laundry, meal preparation and getting to appointments.

The purpose is independence: doing tasks alongside you where you want to build skills, and doing them for you where that is what is needed.

The funding pays for support, not for ordinary living costs you would have with or without a disability. It does not cover:

Food and grocery ingredients for meals · Rent or mortgage payments · Household consumables such as cleaning products · Personal items not related to your disability, such as clothing or electronics.

So a worker's time helping you cook is funded. The groceries are not.

Core Supports is the umbrella budget. Assistance with Daily Life is one category inside it. The others are Assistance with Social, Economic and Community Participation, Consumables, and Transport.

Core funding is usually flexible across those categories unless your plan states otherwise, which means many participants can move funding between daily living support and community access as their needs change through the year. Check with your plan manager if you are unsure whether your plan is flexible.

The level and cost of support in a plan depends on each participant's circumstances and support needs. Some participants have complex needs requiring a high level of daily support, while others only need assistance with certain tasks.

The NDIA decides the amount at your planning meeting based on what is reasonable and necessary for you, which is why two people with the same diagnosis can hold very different budgets.

Standard personal care in Victoria is claimed at the national limits in the NDIS Pricing Schedule 2026-27, effective 1 July 2026:

  • Weekday evening 01_015_0107_1_1 $81.07 per hour
  • Weekday night 01_002_0107_1_1 $82.57 per hour
  • Saturday 01_013_0107_1_1 $103.54 per hour
  • Sunday 01_014_0107_1_1 $133.50 per hour
  • Public holiday 01_012_0107_1_1 $163.46 per hour
  • Night time sleepover 01_010_0107_1_1 $311.79 per night

These are maximum limits rather than set fees, and they are reviewed annually. Verify current figures at ndis.gov.au.

Yes. High intensity supports are for participants whose care involves complex health related needs and requires workers with additional training. They are claimed under separate line items, starting at 01_400_0104_1_1 for weekday daytime at $79.60 per hour under the 2026-27 schedule, with higher limits for evenings, nights, weekends and public holidays.

Call us and we will confirm which level your plan funds and whether we can staff it in your suburb.

Yes. A night time sleepover is claimed as a flat per night item (01_010_0107_1_1), where a worker stays overnight and is available if you need them.

Where you need a worker awake and actively supporting you through the night, that time is claimed at the hourly rate for the day and time it is delivered. We can also provide 24 hour support during a short term adjustment period, such as after a hospital stay.

Yes. We provide hospital discharge support at short notice, including 24 hour care through a short term adjustment period, longer term high level support where it is needed, assistance with medication, mobility and household tasks, and coordination with your treating health professionals so care continues after you get home.

Call (03) 9874 8628 as early as you can once a discharge date is being discussed, so rostering has time to work.

Everyday costs stay with you: groceries, entry fees, rent and personal items are not NDIS funded.
