Understanding the Support

What is NDIS Support Coordination?

Support coordination is funded help to put an NDIS plan into action. A coordinator works alongside the participant to turn the amounts written in a plan into actual services: explaining what each budget can be used for, finding providers with availability, getting agreements and service bookings in place, and stepping in when something is not working.
It sits in the Capacity Building part of a plan, in its own Support Coordination category, and it appears as a stated amount. That matters for two reasons. It cannot be moved into another category the way Core funding often can, and anything left in it at the end of the plan period does not roll over.
Connect Support Group is a registered NDIS provider (Provider Number 4050087037) based at 2/31 Redland Drive, Vermont VIC 3133. We work with participants across Victoria, and remotely Australia wide, and our team includes staff fluent in Mandarin and Cantonese.

The first job is usually translation: what the plan says, and what it actually allows you to buy.

Our Services

What a Support Coordinator Actually Does

Coordination is practical work, not advice in the abstract. These are the four areas that take up most of the hours.
NDIS Support Coordination

Making Sense of the Plan

Going through the plan page by page so you know what you hold, what it can buy, and what it cannot.

NDIS Support Coordination

Finding and Setting Up Services

Shortlisting providers who actually have capacity in your suburb, then getting the paperwork done.

NDIS Support Coordination

Building Your Own Capacity

The aim is that you need us less over time, so we teach the process rather than keeping it to ourselves.

NDIS Support Coordination

Records and Plan Reassessment

Evidence collected across the whole plan period is stronger than evidence assembled the week before a meeting.

NDIS Pricing Schedule 2026-27

The Three Levels, and What They Cost

The NDIA sets both the level of coordination and the number of hours in your plan, so the level is not something a provider picks. Support coordination prices were held for 2026-27 at the Level 2 and Level 3 amounts, while Level 1 rose in line with support worker wages. Victoria sits in the national pricing column.
Level of coordination Line item Who it is usually for National price limit
Level 1: Support Connection
07_001_0106_8_3
Shorter term help to understand a plan and connect with a few supports
$80.06 / hour
Level 2: Coordination of Supports
07_002_0106_8_3
Ongoing coordination across multiple providers, services and informal supports
$100.14 / hour
Level 3: Specialist Support Coordination
07_004_0132_8_3
Complex or high risk situations, delivered by a suitably qualified practitioner
$190.54 / hour
Coordination linked to employment goals
10_002_0106_8_3
Coordination of Supports claimed against employment related funding
$100.14 / hour
Individualised Living Options, exploration and design
01_850_0106_1_1
Working out a home and living arrangement before it is set up
$100.14 / hour
Support coordination is not plan management. A coordinator works on the supports, a plan manager handles the money. Plan management is funded separately, at a monthly fee of $104.45 under item 14_034_0127_8_3, and many participants have both. Coordination funding cannot be used to pay for the supports themselves, only for the coordination work.
Important: Figures above are national price limits from the NDIS Pricing Schedule 2026-27, effective 1 July 2026, which replaced the former Pricing Arrangements and Price Limits document. They are maximums rather than set fees, and they change annually. Confirm current limits at ndis.gov.au/providers/pricing-and-payments/pricing/pricing-arrangements or with your plan manager. Connect Support Group confirms your rate and the hours being claimed in a written service agreement before work begins.
NDIS Support Coordination
Most coordination requests come at a change point: a new plan, a plan review, or a support that has stopped working.

When Coordination Earns Its Hours

The Moments a Coordinator Makes the Difference

Coordination is worth the most at the points where a plan is either brand new, has just changed, or has stopped working. If you are in one of these situations, it is worth a call even if you already have a coordinator.

1

A first plan with nothing in place yet

Funding approved, no providers engaged, and no clear idea where to start.

2

Funding that is not being used

Months into a plan with budgets barely touched, and no roll over at the end of the period.

3

A provider arrangement that has broken down

Shifts not covered, poor fit, or a service that has given notice and needs replacing.

4

Leaving hospital or moving home

Several supports needing to start at once, on a date that is not negotiable.

5

A plan reassessment coming up

Evidence and reporting pulled together from across the plan period, not from memory.

Our Process

How Support Coordination Gets Started

Three steps from first phone call to services running, with the hours and the rate recorded in writing before any coordination is claimed.

1

Free Consultation

We look at your plan with you, confirm what Support Coordination funding it holds, and talk through your goals, your current supports and any language or cultural preferences. There is no cost and no obligation.

2

Plan Implementation

We set out the level, hours and rate in a written service agreement, then get to work: shortlisting providers, checking availability, setting up agreements and bookings, and sequencing what starts first.

3

Coordination and Review

We stay in contact as supports run, track how the budgets are tracking against the plan period, deal with problems as they come up, and keep the records that make your next reassessment easier.

NDIS Support Coordination
The level, the hours and the rate are agreed in writing before any coordination is claimed.

Where we also deliver supports, we say so

Connect Support Group provides other NDIS services as well, including Assistance with Daily Life and Social and Community Access. That creates a conflict of interest that has to be handled openly rather than quietly, so where a coordinator suggests one of our own services, we put that link in writing and give you alternative providers to consider.
The NDIS Practice Standards and the NDIS Code of Conduct require registered providers to identify and manage conflicts of interest and to act in the participant’s interests. Choosing us as your coordinator does not commit you to using us for anything else, and you can change coordinator at any time without giving a reason.

Accessing Our Services

Is Support Coordination in Your Plan?

Coordination is only available where the NDIA has funded it. Here is how to check, and what we need to know before we start. If you are not sure what your plan holds, send us the Capacity Building page and we will read it with you.

What we look for before starting

No coordination funding in your plan? It cannot be paid from another budget, because Capacity Building amounts are stated rather than flexible. The case for coordination is normally raised at your next plan reassessment, and we can talk through what evidence usually supports that request. Call (03) 9874 8628.

Questions Participants Ask

Frequently Asked Questions

Straight answers on what coordination covers, what it costs and how to change providers. If your question is not here, call (03) 9874 8628.

A support coordinator helps you put your plan into action. That means explaining what each budget can be used for, finding providers who have availability and suit you, setting up service agreements and service bookings, sorting out problems when a support is not working, tracking how funding is being used across the plan period, and preparing evidence and reporting ahead of a reassessment.

A coordinator does not deliver the supports themselves, and does not pay invoices. Paying invoices is the plan manager's job.

Level 1, Support Connection (07_001_0106_8_3) is shorter term help to understand a plan and connect with providers.

Level 2, Coordination of Supports (07_002_0106_8_3) is ongoing coordination across multiple providers, services and informal supports.

Level 3, Specialist Support Coordination (07_004_0132_8_3) is for complex or high risk situations and is delivered by a suitably qualified practitioner.

The level and the number of hours are set by the NDIA in your plan. A provider cannot choose the level for you.

Under the NDIS Pricing Schedule 2026-27, effective 1 July 2026, the national price limits are $80.06 per hour for Level 1, $100.14 per hour for Level 2 and $190.54 per hour for Level 3.

These are maximum limits rather than set fees, and Victoria sits in the national pricing column. The cost is paid from your Support Coordination funding, so there is no out of pocket cost for the coordination itself.

No. It is funded under Capacity Building, in its own Support Coordination category, and it appears in your plan as a stated amount.

Two consequences follow. It cannot be moved into another category the way Core funding often can, and anything left in it at the end of the plan period does not roll over. Unused coordination hours are worth using rather than saving.

Look in the Capacity Building section of your plan for a Support Coordination line with a dollar amount next to it. If there is an amount, coordination is funded and you choose which provider delivers it.

If there is no amount, coordination is not funded in your current plan, and the case for it is normally raised at your next reassessment. Send us the relevant page and we will tell you plainly what it holds before you sign anything.

Yes. Coordination can be used whether your plan is NDIA managed, plan managed or self managed. The only difference is how invoices get paid.

If your plan is NDIA managed, your coordinator must be a registered NDIS provider. Connect Support Group is registered, provider number 4050087037.

A support coordinator works on the supports: which providers, what services, how the plan gets used, and what to do when something is not working.

A plan manager handles the money: receiving invoices, paying providers from your plan and reporting on the budget. Plan management is funded separately at a monthly fee of $104.45 under item 14_034_0127_8_3. Many participants have both, and they are not in competition with each other.

Yes. NDIS participants choose their own providers and can change coordinator at any time. You end your existing service agreement according to its notice period, sign a new agreement, and the service booking is updated.

NDIA approval is not required, and you do not need to give a reason.

It is a conflict that has to be managed and disclosed rather than hidden. Connect Support Group also delivers Assistance with Daily Life and Social and Community Access, so where a coordinator suggests an in house service, we put that link in writing and offer alternative providers so you have a genuine choice.

The NDIS Practice Standards and the NDIS Code of Conduct require registered providers to identify and manage conflicts of interest and to act in your interests. Choosing us as coordinator does not commit you to using us for anything else.

Yes, and it is one of the most useful parts of the service. A coordinator keeps records of what supports were delivered, what worked, what did not, and what changed, and can prepare a report for the reassessment setting out what you need next.

Evidence gathered across the whole plan period is far stronger than evidence assembled in the week before a meeting.

We are based at 2/31 Redland Drive, Vermont VIC 3133 and work with participants across Victoria, with remote coordination available Australia wide.

Much of coordination is done by phone, email and video, so where you live does not limit whether we can help. In person meetings are easiest across greater Melbourne, including Vermont, Box Hill, Doncaster, Ringwood, Blackburn, Nunawading, Mitcham, Forest Hill, Croydon, Bayswater, Wantirna, Knox, Rowville, Boronia and Scoresby in Melbourne east, Kew, Hawthorn, Richmond, North Melbourne, Parkville, Carlton, Fitzroy, Collingwood, Abbotsford and Camberwell in inner Melbourne, and Glen Waverley, Mount Waverley, Dandenong, Springvale, Clayton and Oakleigh in the south east. We also coordinate for participants in regional Victoria, including Warrnambool.

Call (03) 9874 8628 to confirm availability in your area.

Yes. Our staff are fluent in Mandarin and Cantonese as well as English, and our support coordination brochures are available in English and Simplified Chinese.

Coordination funding pays for the coordination work, not for the supports being coordinated. It does not pay for a support worker's hours, therapy sessions, transport fares or equipment, and it does not pay invoices or manage your budget, which is plan management.

A coordinator also cannot increase or approve funding. Those decisions sit with the NDIA.
